2024 Recognized Dressage Shows

All signatures (rider, trainer, owner, coach, parent) must be filled out and in black/blue ink
- signed federation entry form
- signed USEF waiver and release of liability (if showing both days you can sign one waiver, but you must write both show dates on it)
Vaccination record (in accordance with USEF GR 845) must be submitted with entry
Negative coggins within 12 months of show date must be submitted with entry
Emergency number must be listed
USEF & USDF membership cards (use eqverification.org) for horse, rider, owner, and trainer (and coach, if applicable) must be submitted with entry (Note: horse show office automatically generates membership cards, but you are responsible for making sure that they are all present and correct)
Payment: credit cards only accepted with online entries- horseshowoffice.com
Additional documents required if applicable:
- For MFS, must include a copy of a 63% at highest test of the level (60% for FEI)
- For Para, must include copy of official dispensation certificate
- For leased horses, proof of lease agreement on file with USEF and USDF
